Form 1 - Application by Physician for Psychiatric Assessment - Ontario Mental Health Act

The Form 1 allows any physician in Ontario who has personally examined a person within the past seven days to make an application for psychiatric assessment at a Schedule 1 facility based on concerns related to serious harm in the context of mental disorder. The form authorizes the apprehension of the person named and their detention in a psychiatric facility for a maximum of 72 hours.
