This evidence brief aims to inform deliberations that could help to improve the quality and experience of hospital-to-home transitions for older adults with complex health and social needs (and their caregivers) in Ontario. In doing so, it mobilizes the best available evidence to identify: 1) the challenges in improving hospital-to-home transitions; 2) three elements of a potentially comprehensive approach to address the problem; and 3) key implementation considerations for these elements.
