Transitions of Care: Leaving the Hospital

Transitional care is an often overlooked aspect of the health care system. As patients move from hospital to home and everywhere in between, the goals of transitional care is to promote the coordination of health care services, ensure continuity of care, and make sure patients are informed and safe when they make transitions. This interactive eModule explores transitional care through four main objectives: (1) defining transitional care and why it is important, (2) describing how transitions affect quality of care and patient safety, (3) preparing a patient for a safe transition, and (4) writing targeted and high quality discharge summaries. There is a particular emphasis on who needs to be involved (the patient, family and family physician) to ensure optimal transition from hospital to home.
