E-Ageing: Rehabilitation Module

This article follows the journey of Mr. Tanner, a patient recovering from a recent stroke, and explores the multidisciplinary approach to care in a rehabilitation centre. Special emphasis is placed on the involvement of the patient and their family in effective rehabilitation goal setting. In addition, the specific roles of various health care providers (Nurses, Physiotherapists, Occupational Therapists, Speech Language Pathologists, Dietitians, Psychologists, Social Workers, Pharmacists and Physicians) are explored in terms of their assessment and management strategies. Weekly team meeting notes are provided, and readers are able to follow Mr. Tanner as he progresses through the rehabilitation program.
