This article follows the
journey of Mr. Tanner, a patient recovering from a recent stroke, and explores
the multidisciplinary approach to care in a rehabilitation centre. Special
emphasis is placed on the involvement of the patient and their family in
effective rehabilitation goal setting. In addition, the specific roles of
various health care providers (Nurses, Physiotherapists, Occupational
