Preparing for discharge can be a time of significant distress for all patients, especially patients in geriatric populations who may have had a difficult or prolonged stay in hospital. This resource provides a quick reference card for care providers as they prepare their patient for discharge. Emphasis is placed on the 5 D’s of discharge planning (Destination, Drugs, Durable Medical Equipment, Dates of Follow-up Appointments, and Drivers) all of which must be considered in order to facilitate safe transitions between different levels of care.
